Postal Applications For People with a Disability

If you are elderly or if you have a disability and you cannot easily get to a Rothercard issuing point you can apply for a Rothercard by post.

Please complete and return the application form to Rothercard Service, C/O Civic Building, Walker Place, Rotherham, S65 1UF

. Please do not fill in the 'Staff Use Only ' Box.

Before returning the completed form please check that you have remembered to:-

include a recent passport size photo
of yourself. We will accept a family photo or your previous Rothercard photo if it is a good likeness.
include proof of age if you are over 60 (as stated on the application form). You are entitled to a free Rothercard if you are over 60 whatever your income. You will only need to send us proof of your age when you first apply for your Rothercard, which is valid for five years.
include proof of income as stated on the application form. All items of proof of income will be returned with your Rothercard. We do not need to see any further proof of income if you are receiving Housing Benefit or Council Tax Benefit.

sign the declaration

at the end of the application form.

include a stamped addressed envelope
